Summary

Remove unused facilities_ppms_suppress_community_care flipper. Includes removing Cypress and unit tests that tested this functionality with the flipper on and off.

Select Community providers (in VA's network) for Facility Type
When the Service type field is available, type a service type (I used Dentist)

@eselkin Do you know if we've recently used that flipper in the event of PPMS weirdness?

No. We don't use it during the PPMS weirdness because that weirdness only lasts like 20 minutes.
