Add issue and PR templates #4

I would like to have templates for submitting issues and PRs. We could look at code.mil for how they do it if we want.

Seems like a good idea. There are some base standard ones Github provides. I don't have a strong opinion but maybe @jhackshaw-dds has thoughts. Trying to keep this clear and open as an OSS project.

As far as Pull Requests, you could include categories for changes made and testing conducted. Code.mil does this here.
