edit_admin_entry doesn't remove htaccess entries

i accidentally banned one of my addresses forever ;_;

also, the wakaba version of that function seems to add more information to the staff log

I was thinking, can we just rewrite the entire file?

Like I can't seem to keep anything in the htaccess because it get rewritten at some point (race?).

If we just redid the entire file to begin with we could allow for a .htaccess-header and I can just add my crap there and never have to worry about it. Also the DB and the file could never become out of sync.
