Returns a Failed to load resource: the server responded with a status of 500 (Internal Server Error: An error occurred (23000): SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry 'flagging-8-0-0-und' for key 'PRIMARY') flag.js:33Unable to save response child

Probably better to just have an "email us a problem" form or use a little embedded wufoo form or something like this that posts to Slack: https://stacktodo.com/tools/form
