[feat] Add changes on the form

Clarification: This issue is created for class4 homework.

Description: For educational purpose, create a minimum modification on the main form to create a new commit, push and merge.

Acceptance criteria:

• create a user on github
• install git
• create a repository for this homework
• clone repository and add minimal changes on the project
• share this repository
