I followed the instructions from the README and this page: https://developer.salesforce.com/page/Lightning_Open_CTI
But I don't see the Phone icon appear in utility bar. Do you know the possible reason?

You can see that Rich Text is there while Phone is not there.

I forgot to add the current user to call center.
