The Tulsa Developers Association aka “Tulsa .NET User Group” is a special interest group focused on continuing education of programmers. We meet once a month and have at least one presentation with topics that are on things like new technologies, methodologies, design patterns, etc. to help programmers mature in their career. Sponsorship is vital to the group’s existence and allows the membership to remain free. We allow anyone, regardless of level of skillset to attend our meetings without cost.
We have a sponsorship level that covers the cost of Food and Venue. An established regular room scheduled provides consistency for members to never worry where the next meeting takes place. And, the food is a great bonus. Members can enjoy time eating and networking with others. This allows for many opportunities to develop that might not have otherwise. Prizes are another added bonus. At the end of each meeting we hold a drawing where attendees can receive a prize. Some prizes have included headphones, USB battery chargers, software subscriptions, and even vouchers to training websites.
The group has overhead that need to be addressed annually. These include expenses like meetup.com for event hosting and domain name registrations. Monetary sponsorship is needed to cover these ongoing costs.
Sponsorship Levels: Gold – $600 or more annually Includes everything from below. A representative from a sponsor may address the group for up to 10 minutes each month. This allows sponsors to let the attendees know more about them and how they’re helping in the community.
Silver - $400 - $599 annually Includes promotional item(s) from Bronze level. Company is mentioned at the beginning of each monthly meeting during the introductory time to the group.
Bronze - $200 - $399 annually Company logo will be on the user group websites and email blasts to the group.
*The Gold sponsor for Food and Venue is a yearly commitment.

Happy to help, but need clarification. I usually do the middle-tier or backend, so not particularly good at GUI design. I can throw up a static page with this content. Did you want to include a form to allow someone to sign up as a sponsor? What do you want to do with the signups?
