Duplicate files #1

With a flag to track which Realm we are in, the two TCOMBAT files could easily be made identical, the only difference is in how we chain back to TOWN. We should try to consistently have duplicate file names on different disks only when the files really are identical.

All finished, now TOWN, OUTSIDE, TCOMBAT are the same file in any Realm. Also general cleanup in the process. However having done this we might as well cleanup DUNGEON and COMBAT in the same way, so leaving open.

Retire distinction between COMBAT and TCOMBAT, no more confusing duplications.

In terms of deployment, files may still be duplicated across floppy disks, in order to spare the user from disk swapping.
