clean up use of sample_rate #6

handling of sample_rate is all over the place:

hardcoded 44100
args.sample_rate in some places
APP->engine->getSampleRate() in just a couple.

It should be cleaned up so that:

all modules that need sample_rate get it during construction
changes to sample_rate are handled with minimal overhead
nice to have: make it glitch free...
