It's crucial to make sure the "Links" in:

are all under blog.bossylobster.com
.
Currently half are also under dhermes.github.io
. I visited https://bossylobster.disqus.com/admin/discussions/migrate/ and clicked "Start Crawler" to follow redirects.

UPDATE: It was as simple as clicking my name and then "Open Thread" on the page.
UPDATE: All the posts where Disqus does not appear have a single quote and have broken Javascript. For example:

Closing, but 10 of the 36 have http
instead of https
as the scheme even though I uploaded a CSV with a URL re-mapping. This is unlikely to matter, but still frustrating.
I think Disqus batches work like this and it takes days to come off the queue, so who knows what has been processed. The new links may be the result of asking them to crawl my pages for redirects 3 days ago.
Only 4 pages have migrated comments (non-G+, wish I would've never switched since those live in a silo) and all 4 have those comments showing up.
Everything is good now :)
