Googlevideosearch.pm reports no form with the requested fields; download fails #389

What steps will reproduce the problem?
1. Install Debian or Ubuntu (tested with Debian Stable "squeeze", Ubuntu 12.04 
and 10.04.4)
2. Update to current packages (apt-get update & apt-get upgrade)
3. Install get_flash_videos (get-flash-videos_1.24-1_all or 
combined-get_flash_videos-1.24)
4. Attempt to download a video via search (e.g. "get_flash_videos youtube music"

What is the expected output? What do you see instead?

Expected output: List of search results, selected video(s) download.

Actual Output:
No menu of search results appears. Instead, the following error is printed:

get_flash_videos:
There is no form with the requested fields at 
/usr/share/perl5/FlashVideo/Site/Googlevideosearch.pm line 15
Searching 'FlashVideo::Site::Googlevideosearch' failed with: There is no form 
with the requested fields at /usr/share/perl5/WWW/Mechanize.pm line 1827.

combined-get_flash_videos-1.24:
There is no form with the requested fields at 
/home/johnnyinternet/bin/combined-get_flash_videos-1.24 line 12742
Searching 'FlashVideo::Site::Googlevideosearch' failed with: There is no form 
with the requested fields at 
/home/johnnyinternet/bin/combined-get_flash_videos-1.24 line 11742.

Debian Stable 'squeeze' produces the below error: 
here is no form with the requested fields at 
/usr/share/perl5/FlashVideo/Site/Googlevideosearch.pm line 15
Searching 'FlashVideo::Site::Googlevideosearch' failed with: There is no form 
with the requested fields at /usr/share/perl5/WWW/Mechanize.pm line 1882.

What version of the product are you using? On what operating system?
I have tried get-flash-videos_1.24-1_all and combined-get_flash_videos-1.24
Problem reproduces on Ubuntu 12.04 and 10.04.4, Debian Stable "squeeze"

Please provide any additional information below.
If you have the Hulu plugin installed, you may get a menu of Hulu results (even 
if you specified another provider, such as youtube or cbs) after the error or 
instead of the error.

Does _not_ reproduce with git clone development version.

Could updated packages be uploaded to the downloads page, or link to the git 
instructions on the installation page?

Fixed. Wiki page updated with git instructions and link to unofficial releases.
