Fixed all the issues in survey-create.html

should be fully functional now

Bugs:

Create Survey button leads to the wrong endpoint
Survey is being saved twice

Currently helping out with debugging

Still need to check if the questions are being saved properly. I'll create an HTML template to view them

The questions in a survey a user is trying to create were not being persisted/saved in the database. Further refactoring and investigation will be done have this feature fully functioning.
