Repeated subforms and example todo app (#91 and #94) #96

Addresses issue #94 Includes an example todo app for issue #91 . You can log in with any given credentials. The earlier examples are preserved by using dashdo to navigate across.

Thanks, this looks great and works. One stylistic change request @timds : you think we can have just one "add new item" button, at the bottom of the list?

Thanks, I've made the change to the button position. This of course means that an item can only be added at the end of the list.
