Create some sort of anonymous salary spreadsheet #17

In response to these comments from @kfrn and in conjunction with @ross-spencer WIP repo we should think about creating an anonymous survey or some sort of anonymous spreadsheet with salary information along with demographic information, like location and gender, etc for our field.

This is great! I wonder if we can harness something like etherpad lite (like maybe ethercalc to gather data anonymously on a on-going basis, although google docs may be the easiest way to pull everything together.
