Add per-link comments functionality #11

It would be great to discuss each link separately
And have a comments count near comments icon

Currently i'm trying to implement it via disqus, but it's not per-link more of a global comments. Done with huge modal window, hacky but works for now :)

Would be great if someone could expand on this.
