log when we get a KeyError on 'name' index for a label #9

1 more instance of the TypeError: string indices must be integers error came up in the logs on the 17th from this PR, but I still couldn't determine from matching up the error to the PR what caused it. So hopefully this log will make it clear next time it happens.
