ux may be considered a bit counter-intuitive with how defaults are currently set up. i understand that it provides more flexibility, but i think users would rather simply have a settings page where they could enter their misc notification details and then you simply select them on the alert. if having the flexibility is important, and advanced/override button could take care of that to set custom notifications.
