Remove field counter? #35

This is very PICA-specific and not used consistently in PICA software anyway (there is both 209Ax00-09 and 209A/$x00-09 to denote the same). Maybe remove from Avram specification and allow as extension? Or switch to the second so we have / as common character to extend the tag?

This may also involve modification of PICA Path by moving xtags to an extension.

As GBV uses syntax with /$x better change the syntax (already anticipated in avram-js).
