Add back deprecated fields, subfields, codes #36

With boolean key deprecated: true instead of former parallel objects deprecated-fields, deprecated-subfields, deprecated-codes. This is much easier to process but it does not support the same field/subfield/code have two distinct definitions (current vs. deprecated definition). I think a boolean key still covers the most common case.

Deprecated fields and subfields (not codes) are required by qa-catalogue, so these should be part of version 1.0.0.
