Add information to address table how inputs are used. #268

For some LUTs the input consists of two quantities. In those cases it would be good to note in the address table in which order these quantities are concatenated to form the input word, i.e. whether quantity1 is the most-significant or least-significant word.

We won't put this information into the address table, but document it separately.
