Add a form "Report a bug" #150

The idea here is to provide a way for the users to report a bug. If we had a form on the website, it must be targeted by the "Help" ("Aide") section on the extension.

Mind these URL parameters here, this link can be pre-configured with a title, labels, etc... So this can be used in conjunction with a form or something, but I think a simple link with the bug report will do fine for now...

Well, in fact, I think we should extends this to feature requests as well: You have a great idea for Dismoi? Comme and tell us!
