Why It's Good For The Game

You can see in the description of the lowborn perk that it implies the people who have it are downtrodden, poor, and seen as filth. This is not really common because they get lost in the crowds, this lets them have an extra description line if their face is uncovered. Likewise with nobles.

Having only a third of your sanity remaining is visible to others if your face is exposed, dilated pupils, shifty eyes, weird cast to face.

Having less than five percent of your sanity left is very easily noticeable, as well, in the same conditions.

In any case, wearing a mask makes all of these not noticeable.
