It seems I found out the reason why I get multiple identifiers: I use a plugin called "Public Post Preview", and it mistakenly show a Disqus comment when our authors visit the preview page. If Disqus can be disabled on these public preview links I think I won't have this problem in the future.
I am not using this and still my commets are duplicated.
