This PR implements an api to add/remove existing mod values to existing list entries.

This includes:

Database migration with a new aid__list_entries__mod_values table.
new EntryMod Ecto schema.
new add/remove functions in Aid phoenix context.
new GraphQL types and mutations to add/remove existing mod values to existing available/needs list entries.

Fixes #62
