This also adds the ability to add configuration options to conf.py and / or
the asciinema directive.
In addition I changed the author field -- I'll leave that to you how to fill that in.
Note that I also changed the installation instructions, as the original ones weren't valid for the fork. These will need to be changed back if this gets merged and the PyPI package updated.
Btw, I started writing this under the name sphinxcontrib.asciinema-local, but figured that it would be better to integrate it upstream. Having two different asciinema sphinx extensions didn't make much sense :).
