please style add exercise form #59

please login into mpa.jsoft.com.au

go to setting -> my exercise -> add exercise

you will see form look messy. i added group of checkbox , some radio box line too. can you style it to make it look better ?

or you can add group checkbox like i did on the form, i will apply it to live form then.
