hi ihor can you make these two pages into nice design pages that we can send clients of MPA to via a private message in the mobile chat? https://www.worksafe.vic.gov.au/__data/assets/pdf_file/0004/208543/NOC-The-lower-extremity-functional-scale-2006-11.pdf https://www.worksafe.vic.gov.au/__data/assets/pdf_file/0020/208541/NOC-The-upper-extremity-functional-index-2006-11.pdf we need to be able to have them answer online, and then gather the data.

Can you add button submit at bottom page ? Else user can not submit it

button added, but there are no form tag, so you need to use your own handler
