Historical note should be an appendix

The current "historical note" is an interesting curiosity, but it probably belongs in an appendix section, not in the current prominent place.

Note that #8 recommends some concrete guidance that might be better off in the current location of the "historical note"

I have simplified the Historical Note a bit and will try to resolve #8; however, there was an earlier comment that the historical information was not sufficiently prominent when it was at the end of the document so I earlier moved it twoards the start of the docuents.
