Cross-referencing #33

Full cross-referencing, including from e.g. argument types in function signatures, etc.

Needs a pass to generate lists of all documentable items. Can reuse visitor.Item, just with some tweaking and a separate pass to accumulate all items.

Fixed by 59c8c9b369ebb0b2b29e22a6b6d159a8b20b8b2. Not perfect yet but mostly works.
