Recent changes to the zindex of the header caused the persistent ToC to render incorrectly. Fixed with a couple more zindex settings.
Also removed the 'highlight line' highlighting from the programlisting example. Highlighting lines and syntax highlighting don't play well together.
This turned into a mess.
