Small fixes #368

  1. Fix a problem in the persistent ToC where the ToC icon was hidden behind the header
  2. Remove line highlighting from "Five Interesting Things". Line highlighting and syntax highlighting do not play nicely together.
  3. Fix the expected results for a couple of tests.
  4. Improve the diagnostics for failed CALS tables. (table-cals.049.xml fails in CI and I'm trying to work out why; almost certainly memory but I'm surprised by the failure message.)
