Move Description to end of PULL_REQUEST_TEMPLATE #59

Enhancement

On the PR template, we should move the Description section to the very end of the template, because GitHub will automatically append the commit description that the PR is being raised against, and essentially makes it easier when raising a PR to not have to copypasta stuff around.

There are a few other minor enhancements we can make as well to ISSUE_TEMPLATE

Also we can move CONTRIBUTING.md to .github
