No delete cascading for appointments

After creating an appointment with dentist Alex Yeoh and Patient Alex Yeohh, deleting the patient or the dentist should delete the appointment as well. Without deleting the appointment, the filter-appointment command can filter-appointment dentist 1 even though there is no more dentist 1 as it has been deleted.

This can be an inconvenience to users.
