Same issue on a MBP 4.1 with 10.15.4 (fresh install with the latest patcher). Similar behaviour also with other non Safari plugins like Webex Productivity Tools for MS-Outlook.
same with me! You guys are great with this patcher. I have uninstalled 1Password7. I hand deleted from the Library folder the 1Password Cache as well as the preferances folder. after reinstalled 1Password7 I tried selecting (activating) the installed 1Password safari extension in safe boot mode and and regular boot mode.
Ps. I have 2 unpatched Catalina's running on more current machines and the activation works well on those 2. mid2012 MBP and a 2015 Base MacBook.
My Patched machine is a mid 2010 iMac 11,2 with ATI Radeon HD 4670 256MB.

Same problem again on a MBP 8.2 with the latest Catalina Patcher 1.4.1. According to Apple this can be solved if you do a safe mode boot, but it seems that this is not possible on an unsupported mac (due to conflicting boot-args). Any ideas?

I installed this package and the safe Mode works on my not supportet Catalina Mac. But I can't enable Safari Extensions in the Safe Mode.

Also having this issue with a Mac mini 5,1 and Catalina 10.15.7. Every time I try to enable an extension in Safari either in normal boot or safe boot it gives a popup that says "Safari detected an app or service that interfered with clicking."
I thought maybe the issue was that I had Logitech Options installed but my Air that has Catalina installed (non-patched) does not have this issue.

@Rikchell in my MacBook Pro with Catalina 10.15.5 there is no folder name AppExtensions
in the following directory
~/Library/Containers/com.apple.Safari/Data/Library/Safari/
How can I solve the issue?

The AppExtensions folder does not exist in my path. I have tried installing Grammarly, Enpass but without success...

Try installing the ghost ad block, should create the folder for you.

Problem solved. The issue was with the Catalina version. With 10.15.5 you have safari 13.x, there the folder was not created. After upgrading to Catalina 10.15.7 and updating to Safari 14 I was able to do the workaround suggested by @Rikchell .

Use this tool. It's better than doing it manually. This was developed for Open Core Legacy Patcher which has fixed the issue, but its still useful for dosdude's patcher.
