This updates copyright and code of conduct notices in preparation for inclusion in the .NET Foundation.

As part of this PR or a follow-up, we should also:

[x] Add code signing (when code signing cert is issued)
[x] Update repo-referencing links (cba92f1711e3b18b504306e0d2f593ce3c25197d)
[x] Move the repo to the dotnet org.
[x] Notify gitter.im to move discussion to new URL.
