@JCox-Williams Just a few questions for you:
Can you share the map with me?
How was your relationship configured? i.e. using GlobalID/GUID fields, or user-defined fields for the primary/foreign key fields.
Is your relationship between feature and table or feature and feature?
I am unable to share the map outside the enterprise account, I am only a publisher and not an admin so i cannot change that setting.
I have a relationship class, one to many, that was published in a FGDB with the standalone table and the points FC it is related to. This method has worked fine with current Collector.
The relationship is between a point feature service and a standalone table.
The point feature service is the ONE and the standalone table is the MANY. There is a unique ID which is a site ID assigned for each point, each record in the standalone table uses the site ID to relate it to the location.

@JCox-Williams what version of Portal/Enterprise are you working with?
Did you publish from ArcMap or Pro?
Was it published as a hosted feature service or a federated ArcGIS Server feature service?
This was published to ArcGIS Online, using a zipped FGDB and publishing it within ArcGIS Online. I used the same web map and data in ArcGIS Online with current Collector and was able to add new related records.

Thanks for the data @JCox-Williams. I was able to reproduce the issue.
I don't think this issue is related to that data specifically, but rather how it was published. If you publish it from ArcMap instead of as a zipped FGDB, it works fine in Aurora.

But that same publishing method works just fine in the current build of Collector. If you don't want to support that publishing method going forward, that's fine, but it was something I have done in current Collector but was not able to do in Aurora so I reported it.
Hi @JCox-Williams it is something we want to support. I was just sharing my testing results and explaining that its not data specific, but publishing specific. We're still investigating on our end.

Issue
I am unable to add new related records for a location, this functionally exists in current Collector.
Expected Behavior
I have a standalone table with surveys for specific facilities I am trying to edit. When I select a specific facility and bring up the details pane, under the relationship I have setup based on site ID I can view related records or create a new one. In current Collector I can select "New" and I am able to then populate the attributes and submit the new record.
