Write a README explaining the Turnkey aspects #304

Especially the required SSM params and defaults.

And how to deploy inter-commit.

Also emit the specific required param names from the build, and say in the README how to compute them with stackname.

As part of a getting-started guide, my instructions could be like "clone/fork this repo and run the main or feature-branch build." Then the builds could instruct you to enter AWS credentials if it finds none.

Explain how to wire up a domain name, and how to use all the SSM config options. It would be good to have an npm script or similar print these instructions.
