On a track like https://music.youtube.com/watch?v=UHCWRX_SY68&si=ogc0PbBxD9KaHf0Q where they's a video option. The extension is choosing the video thumbnail over what the YouTubeUI shows as the album art.
The screenshot shows the available Album Art, but the .mp3 is created with the first frame of the video.
