I assume that's just how the creator uploaded it, i.e. an issue with the .puz file, nothing actually messing up with Downforacross itself. I've played puzzles with typos before too, but I'm not sure users/hosts should have agency to "fix" these per se. But a tag to warn others could be nice?
I like the idea of a Report as broken
button, in some menu on the main player interface + Displaying a broken
tag on the homepage.
A more general solution (since broken puzzles are only so frequent) could be an upvote/downvote feature, which would enable hiding puzzles below some score but also helping the well-liked puzzles be discoverable, or feed into some sort of ranking system.
I like the idea of being able to vote on puzzles. Honestly a (minimalistic) commenting system to discuss puzzles could be cool too, but maybe a little out of scope here or too ambitious.
I'd be happy with a broken tag. I worry that voting up/down will promote "known" crossword makers (nyt, etc.) and down vote new ones.

This is obviously a case of an incorrect answer key. It would be nice if there was a way to report incorrect puzzle answers.
