More kerning #12

The kerning pairs in the original font suitcase files are fairly limited in scope, so this should be fixed sometime after the main bulk of the 2.0 project is done. There is almost no kerning for accented characters (though this might get partially fixed for 2.0), or for punctuation characters (either with each other or with letters).

The original fonts were also incapable of having kerning pairs between small capitals and regular capitals, since small caps were in a separate font. This probably needs to be fixed most urgently.

Also the kerning is somewhat inconsistent even when it is there: Ly is kerned in the bold font, but not in the Roman one, to give just one example.
