status of bookbag form #12

Will we be able to recreate bookbag form or not?

des plaines public library Form's theme cannot be changed because it is an older version of Google forms. Asked Sara and Emme if they want to recreate form or link to it as a separate thing. This is what it would look like embedded in current site.
