HTMLFixer needs CSS improvements #118

HTMLFixer currently leaves too much CSS in place in fixed files.  Width and
height elements at any CSS level can throw off the layout of the page and
force scrolling.  HTMLFixer should remove all height and width in style
blocks and per-element style attributes.  Other attributes may need to be
removed as well

HTML support will be deprecated in 2.0.  Either fix HTML on PC or use Plucker 
format.
