Fix some issues #35

I saw that the tree did not implement some commits from the forks tree. I merged @planio and @fschupp branches on the master and added a fix for Redmine 3.

For the simplecov gem, I'm not sure the version fits perfectly to Redmine <3, you can change it if you like.

This PR also fixes #34

Thanks for the effort, but this is not in a mergable state. Most of these commits are obsolete or plain wrong. Also it's by for not enough to make it work in Redmine 3.

But no worries, I just fixed Redmine 3 support myself and release v0.5.
