Implement new out-of-band setup protocol #24

Try implementing new setup protocol.

Try (again) to get responses from RIPE and APNIC on whether they are ok with this as a replacement for the kludgy old protocol that "just grew".

Leif suggests that XML might even include comments suggesting what to do with a particular XML file. This doesn't play so well with the notion of this as an interoperable thing, but comments should be harmless and we can at least document the tools we know about, or give pointers to them, or something.
