Allow users to map players on their uploaded cards #53

Something we noticed in the field was that uDisc exports were not consistent. Some had players full names, display names, or nicknames. We should have the card upload screen be done in steps:

Upload / paste uDisc card
Extract users, attempt to find players, allow user ability to manually select from the system
Submit card
