Should prompt the user for

  1. Establishment Name (via searchable drop down ) Where drop down is a max 5 list of all hospitals queried by search input
  2. Establishment Representative First Name
  3. Establishment Representative Last Name
    • (Table)
  4. Equipment Type
  5. Equipment Quantity

  6. + button to add another row

  7. A reCapcha object
  8. A submit button that is activated on reCapcha submission
  9. Message telling the user It is immoral to claim to represent a doctor/nurse/hospital in a manner that would restrict its ability to receive aid. Click to proceed.
  10. Message telling the user their request has been successful (with all the stuff they requested)
