Annotated PDFs don't work in Adobe Reader #4

Annotated PDFs seem to work fine in Preview on OS X, but an error is detected by Adobe Reader, which then fails to display the annotations.

It's not clear how to debug this as Adobe Reader doesn't seem to provide a debug log... probably best to try not annotating, just copying the existing PDF, and build up from there until the error starts appearing.

Fixed - I was proving an out-of-range parameter to the sc (set color) operator.
