a bug <ruby><rt></rt></ruby> #686

Describe the bug

To Reproduce

  1. open any md file
  2. input \樋口\といくち\\
  3. Then といくち isn't on the top of 樋口. といくち is shifted.

Expected behavior といくち should be on the top of 樋口. In one of the previous version, this format was Okay.
