Corrected the contact form issue where dropzone was the only form field, which was a result of oversights made during the splitting of the primary view into independent page views for both documents and contacts.

Update both corrects the form to reflect all fields expected by model and types, but also updates model and types to accept complex address structure, replacing previous (less useful) string values.
