No change on the layout is expected (based on the Manifesto of the plugin)

The numbering layout has changed, see screenshot (default AMA Style was used) - even after clicking on the Keep as HTML button

Disabling or deleting the plugin will require the same bit of CSS that the plugin uses to be added to your child theme or your theme's custom css. The HTML is intact and is not different than the html that the plugin provides. Unfortunately, some citation styles just can't be achieved without a css suppliment.
